The MOS tube burned without connecting the load, what is the reason?

Everyone help me look at this circuit

If the VGS of AO3415 is ±8V, will this circuit burn?

The originally intended function is to enable 12Vout output when enabled, and no output when enabled.

The previous design is used, and I care too much. Today, I found an AO3415 burned under the condition that the output is not connected to the load. Check the manual to see that VGS is ±8V. Is this the reason?

How to modify it?

It should be the problem of Vgs overrun. If you cannot replace Q1 with another PMOS with a higher Vgs range, you must change the board. Add a series resistor at the D pole of Q2 and divide the voltage with R1 to control the G pole of Q1, so that Vgs is not -12V. Too.

The voltage of VGS has exceeded the specification requirements. Anything that exceeds the specifications requires risks. Suggest:

1. Add resistance to divide voltage to within 8V.

2. Change the MOS tube with larger VGS voltage.

The manual says that VGS is about -4.5V as a normal value, don't exceed it.
